School-Based Health Centers

A School-Based Health Center (SBHC) provides comprehensive primary care services in a school year-round. There are seven SBHCs in the city. A SBHC can serve as a student’s primary health care provider or supplement the services they would normally receive. Each SBHC offers medical, oral, social, and mental health services to any eligible student. SBHC staff can also provide referrals to specialty care, community providers, and other programs that coordinate to ensure every student receives the care they need to lead healthier lives. Free Physicals for Student-Athletes

DCPS high school student-athletes may enroll at any SBHC. They are eligible to receive all services provided. This includes the completion of a Universal Health Certificate (UHC) that is needed to participate in athletics. Students should choose whichever location is most convenient for them to establish a long-term relationship. Each SBHC has a different set of enrollment and consent forms that will need to be completed a parent or guardian. Enrollment forms for each site are linked below. Students must call ahead to schedule an appointment for their first visit.
